Background Little is known about communication between patients, families, and healthcare providers regarding ethical concerns that patients and families experience in the course of illness and medical care. To address this gap in the literature, we surveyed patients and family members to learn about their ethical concerns and the extent to which they discussed them with their healthcare providers. Methods We surveyed adult, English-speaking patients and family members receiving inpatient care in five hospitals in the Washington DC-Baltimore metropolitan area from (...) July 2017 to March 2020. Descriptive statistics were used to determine the frequency, comfortableness, and helpfulness of discussions regarding ethical concerns experienced when sick or receiving medical care. Univariable and multivariable stepwise logistic regression models were used to identify associations between healthcare provider and respondent characteristics and attitudes and (1) the likelihood of speaking to a healthcare provider about their ethical concern and (2) their level of comfort during these discussions. Results Of 468 respondents who experienced ethical issues, 299 (64%) reported discussing the situation with a member of their healthcare team; 74% (197/265) of respondents who had such a discussion found the discussion comfortable, and 77% (176/230) of respondents found the discussion helpful. To make discussions more comfortable and helpful, respondents proposed suggestions in open-ended responses involving (1) content and quality of communication; (2) positive healthcare provider qualities such as empathy, open-mindedness, knowledge, honesty, and trustworthiness; and (3) other contextual factors including having adequate time and available resources. Conclusions Patients and families often have ethical concerns that they discuss with clinicians, and they want clinicians to be routinely receptive and attentive to such discussions. (shrink)

Also explored are several problems concerning the concept of definition, non-designating terms, the interdependence of quantifiers, and the idea of an assertion sign. The contributions (...) concerned with Wittgenstein's investigations into the philosophy of logic and mathematics pursue issues relating to logical necessity, the undeniability of the law of the excluded middle, and the source of self-evidence, often characterized in the literature as the "rule-following considerations". Additionally, they examine Wittgenstein's attitudes towards the very idea of set-theory as a possible foundation for arithmetic. Ce texte de Bernhard Waldenfels est issu de la seconde partie de son livre Socialité et altérité – modes de l’expérience sociale, 363-85), où l’auteur met à l’épreuve et approfondit sa théorie de la responsivité à travers une série de débats avec Husserl, Schütz et Gurwitsch, Searle, Castoriadis, Foucault et Ricœur. Dans sa discussion avec ce dernier, il insiste sur les thèmes du souvenir et de l’oubli, en prenant pour base Temps et récit et La mémoire, l’histoire, l’oubli. Dans les deux premières, Waldenfels retrace les arguments de Ricœur en insistant notamment sur le manque d’importance accordé à l’oubli dans le récit par Ricœur. Dans les deux dernières, l’auteur propose une révision de la philosophie ricoeurienne de l’oubli sous le signe d’une phénoménologie responsive. The precarious rights of senior citizens, especially those who are highly educated and who are expected to counsel and guide the younger generations, has stimulated the creation internationally of advocacy associations and opinion leader groups. The strength of these groups, however, varies from country to country. In some countries, they are supported and are the focus of intense interest; in others, they are practically ignored. For this is reason we believe that the creation of a network of all these associations (...) is essential. The proposed network would act as a support for the already-existing policies of the United Nations’ High Commission for Human Rights, of independent experts, and of the Global Alliance for the Rights of Older People. All three have long ago recommended the creation of a recognized instrument for uniting presently scattered efforts. The proposed network, therefore, will seek to promote the international exchange of relevant expertise, and it will reinforce the commitments and actions that single countries are currently taking to meet these objectives. For example, informative public events can be organised to promote particular support initiatives and to provide an opportunity for new members of the network to be presented. The network will promote health for senior citizens, disease prevention, senior mobility, safe free time for seniors, alimentary education, protection against new risks and dangers, as well as equity in the services necessary for seniors to adopt new information and communication technologies. In the case of retired academic members, the network will promote equality with respect to continuing use of digital technologies (particularly email), continuing access to research libraries, and the guaranteed ability for seniors to fund their own research programs and to deliver free seminars. "Bernard Groethuysen (1880-1946), philosophe d'origine allemande, partage, dès 1904, sa vie entre Berlin et Paris où il étudie la Révolution française et rencontre, grâce à Bergson, Alix Guillain (1876-1951), traductrice et journaliste à L'Humanité. Vivant avec leurs compagnes dans le phalanstère de la rue Campagne-Première, Groethuysen et Paulhan, devenus amis, oeuvrent à partir de 1920 à La Nouvelle Revue francaise, dont ce dernier prend la direction cinq ans plus tard : ils font découvrir Hôlderlin, Kassner, Kafka, Büchner, Musil... En 1927, (...) ils inaugurent leur collection "Bibliothèque des Idées" par l'essai majeur de Groethuysen, Origines de l'esprit bourgeois en France, "notre premier enfant", écrit-il à Paulhan. Dans son ultime lettre d'Allemagne, de 1931, il s'inquiète : "L'Histoire devrait nous donner des vacances. On en profiterait pour reprendre la vie." En août 1932, devant l'essor du nazisme, Groethuysen s'exile en France. Quant à Paulhan, le Front populaire le voit s'engager aux côtés de Jean Longuet, maire socialiste de Châtenay-Malabry. Arrive la guerre : Germaine et Jean Paulhan quittent leur maison, leur jardin, leurs animaux, dont Alix Guillain et Groethuysen, naturalisé en 1938, prennent soin. Après la défaite de l'Allemagne, le philosophe rêve encore d'utopie, mais il meurt le 17 septembre 1946. Les travaux de Groethuysen m'intéressaient, il s'intéressait aux miens", confiera Paulhan à Ponge. Ce volume prouve leur affectueuse amitié, fondée sur le travail en commun pour la NRF. Según Martin Heidegger, la filosofía se consuma en la época actual en el pleno desarrollo de las ciencias particulares. Estas se ocupan del ente. Pero ya desde su inicio pensó la filosofía al ente en tanto ente, ya como fundamento del todo; ya como ente supremo, relegando al Ser. Filosofía y poesía, pensar y poetizar, son modos paradigmáticos del decir, dialogan, se co-responden. En la consumación de la filosofía, finaliza un modo del decir y del pensar, hasta ahora excluyente: el (...) del ente en cuanto tal. En la consumación de éste, acontece la falta de fundamento, percibida como angustia y silencio. Esto leemos en Samuel Beckett. Una temporada en el infierno de Rimbaud dice la historia de Occidente desde el punto de vista del fundamento en tanto ente supremo: esto es, Dios. En el decir de Rimbaud se patentiza el ser religioso cristiano de Occidente. El callar del poeta se corresponde con el ocultamiento del fundamento poéticamente pensado como ente supremo. En Persona de Bergman se representa el drama del silencio como vacío de las palabras, a la vez que como abismo en el sentido de la ausencia del ente supremo en tanto fundamento, experimentado desde la inmanencia de la conciencia, con lo que se suma la problemática del sujeto de la Modernidad, ahora desfondado por la ausencia. Otra raíz tiene en primera instancia el poetizar de Paul Celan: arranca del pensamiento de Martin Buber y tiende deliberadamente al silencio como modo privilegiado de la relación con un Tú que es Otro absoluto. Sin embargo, su pertenencia a la poesía en lengua alemana implica un cuño metafísico y lo lleva a la órbita de la poesía europea finalizante como silencio. El decir debería entonces detenerse en este destino de la poesía occidental: dejar callar al silencio. Oír y escuchar su voz sin palabras. It is necessary to specify in which sense the concept "Silence" is to be used in Celan's poetry. If on one hand it can be said that his Lyric is an answer to Adorno's impossibility of poetry after Auschwitz, and that it arose in the frame of the annihilation of war and Nazism that also devastated language , on the other hand, it is in a dialectical way that this allusion to Silence, neither positively nor factically determinable, could be explored, (...) according to the opposing tension lines that go across Celan's Lyric.Es necesario precisar en qué sentido se habla de silencio en la obra de Celan. Si por un lado puede decirse que su lírica es una réplica a la imposibilidad de la poesía después de Auschwitz y que se alza en el marco de la aniquilación de la guerra y el nazismo que arrasó también al lenguaje , es de modo dialéctico como esta alusión al silencio, nunca positiva y fácticamente determinable, puede ser explorada, según las líneas de tensión contrapuestas que atraviesan la lírica de Celan.
